A reminder to all general and prescribed medical practitioners that current GP management plans and team care arrangements for chronic disease management ceased on June 30, 2025.
This includes:
- GP management plans: 229, 721, 92024, 92055
- Team care arrangements: 230, 723, 92025, 92056
- Reviews: 233, 732, 92028, 92059
From 1 July, GP chronic condition medical plans (GPCCMPs) took their place. These have been allocated new GP and prescribed medical practitioner item numbers, based on whether the appointment relates to preparing or reviewing the plan (in person or through telehealth).
The change comes as the Department of Health, Disability and Ageing seeks to streamline chronic disease management for providers and patients. As a result of introducing the GPCCMPs:
- Medical practitioners no longer need to consult with two collaborating providers
- Allied health providers needn’t confirm acceptance of a referral
- Allied health providers don’t need to help prepare the plan
Practitioners can refer patients directly to relevant services, and allied health providers must only report back to the GP once they have delivered certain services.
Patient eligibility for a GPCCMP remains at the discretion of the medical practitioner. Patients who are registered with MyMedicare must access GPCCMP items through the practice where they are enrolled, and unregistered patients can access them through their usual GP.
